Pre K - 6TH GRADE

1. Complete the Enrollment Packet Online:

• Go to the FACTS Family Portal (You will use the same username and password that you use for your FACTS Family Portal this year. If you forgot this information, there is an option "Forgot username or password" in which you will type in your email address affiliated with your account. If you don't know which email you used to create your Portal, please call the front office. Your account will not be recognized if you do not use the email address that you used to initially create your account)
• Update and submit the Enrollment Packet.

2. Submit the following online or to the school office:

• Updated Certificate of Immunization Form 121
• Parish Affiliation Form
• Baptismal Certificate (if Catholic), if not on file

3. Pay the $400 registration fee to the school office
